WHAT HAPPENED​

On August 18, 2025, during the onboarding process of a new provider, a patient list was mistakenly provided to our practice that included some individuals who were not her active patients. Unfortunately, this information was inadvertently imported into our electronic health record system. As a result, some individuals may have received routine promotional communications that our office sends to our established patients.

WHAT INFORMATION WAS INVOLVED

The information included was limited to demographic details:
• Name
• Date of birth
• Address  
• Phone number

​

NO medical records, diagnoses, treatment information, insurance information, Social Security numbers, or financial account details were accessed or disclosed.

WHAT WE ARE DOING

We immediately identified and removed the affected entries from our system upon discovery. We have:

• Completed a comprehensive risk assessment under HIPAA's Breach Notification Rule
• Strengthened our onboarding and verification processes to ensure this type of error does not happen again
• Retrained staff on HIPAA requirements for patient communications
• Notified all affected individuals by mail
• Reported the incident to the Department of Health and Human Services as required

WHAT YOU SHOULD DO

NO ACTION IS REQUIRED from affected individuals. However, we recommend remaining aware of any unusual calls, emails, or mail you may receive. We have no evidence that any information has been misused in any way.
